2026/2027 Waiver
This is the waiver every player signs once per season, in the player portal, before checking in to play — the text below is exactly what you’ll be asked to read and sign there. It isn’t the portal’s Terms of Service, which covers using the site itself.

Acknowledgment of Risk
I acknowledge that ice hockey is a fast-paced contact sport carrying inherent risks of injury, including but not limited to collisions with other players, boards, glass, pucks, and sticks; falls on ice; strains, sprains, fractures, concussions, spinal injuries, and, in rare cases, catastrophic injury or death. I confirm that I am voluntarily participating in the Ottawa Morning Hockey League (the “League”) with full knowledge of these risks, whether or not specifically listed above, and that these risks cannot be eliminated regardless of the care taken by the League, its organizers, officials, volunteers, or other players.
Release of Liability
In consideration of being permitted to participate in the League, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue the Ottawa Morning Hockey League, its organizers, executive members, coaches, officials, volunteers, rink/arena operators, and their respective employees and agents (collectively, the “Releasees”) from any and all liability, claims, demands, actions, or causes of action arising out of or in any way connected with my participation in League activities, including travel to and from games and practices, whether caused by the negligence of the Releasees or otherwise, except where such liability arises from the gross negligence or wilful misconduct of a Releasee.
Medical Fitness
I confirm that I am physically fit to participate in ice hockey, have no medical condition that would make participation unreasonably dangerous, and have not been advised otherwise by a physician. I understand it is my responsibility to disclose to League organizers any condition that could affect my safety or the safety of others.
Equipment
I agree to wear all equipment required by League rules and Hockey Canada safety standards (including a CSA-approved helmet with full face protection where required) and understand that the League is not responsible for verifying the fitness or condition of my personal equipment.
Indemnification
I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or cost they may incur due to my presence at or participation in League activities, whether caused by my own negligence or that of any other person.
Photography/Media (optional)
I consent to the League using photographs or video of me taken during League activities for promotional purposes, unless I have advised the League organizer in writing that I do not consent.
Term of This Waiver
This waiver is effective for the entire 2026/2027 season and covers all games, practices, and League-sanctioned events during that period.
Governing Law
This agreement shall be governed by the laws of the Province of Ontario. I confirm I have read this agreement in full, understand its terms, and am signing it voluntarily and without any inducement.
Severability
If any portion of this agreement is found by a court to be void or unenforceable, that portion shall be severed and the remainder of the agreement shall continue in full force and effect.
